We are a non-profit organization with a mission to improve quality and reduce cost through coordination of care for the most costly and vulnerable in our health care system.

What we do

We perform our mission in a variety of ways, from developing software and technology solutions for care coordination, to performing data analytics on large diverse datasets for identifying super utilizers, to setting up intervention programs and care plans for clients enrolled in the intervention program.

As a Media & Communications Specialist, you will manage specific projects related to the successful implementation of PCIC’s communications objectives, with a strong emphasis on creating and implementing integrated communications workflows and plans where media outreach is a major element. You will also be responsible for internal team communication workflows as well as event planning and communication with PCIC supporters and stakeholders.

Project Manager

Status: Position filled

Full-Time with Benefits

As a Project Manager, your primary role is to coordinate and develop key projects with partner stakeholders, assist with building PCIC’s research capacity through designing and implementing innovative quantitative and qualitative projects with a focus on High-Need, High-Cost (HNHC) patients, assist with grant funding and legal framework. You will participate in strategic development of the organization by contributing to weekly core strategy team meetings and related activities, and cultivate relationships with external researchers and experts.

Role: Sr. Social Worker

Status: Position filled

Job Summary:

Will coordinate and lead the day to day operation of the intervention team. This position is 75% field based, encompasses managing a small team of care coordinators working with complex clients. The program’s goal is to reduce client usage of emergency room services, and shift towards primary care models.
