Locally and nationally, the three biggest barriers patients face in accessing care are:

Unreliable transportation to and from appointments
Not taking medicine (due to lack of access, prohibitive costs, negative side effects)
Inability to get timely appointments

In Harris County, there are 6,000 patients with complex needs whose cost of care has exceeded $800M for Medicaid alone.

Providers are unable to share patient data across agencies, leading to a lack of coordination and accountability across systems.

PCIC leverages data to improve community-wide care coordination. Our work:

Builds care plans around which values and goals are most important to the client.

Links client records across medical and non-medical agencies so patient records and care plans build from one visit to the next.
