Research and Development

Data and Evidence Drive our Practice

Our Research and Evaluation team is tightly integrated into our Care Coordination and Technology divisions, providing internal value by developping evidence-based evaluations and faciliating meaningful feedback between teams. Externally, we partner with well-known health systems in Houston to leverage a universe of data and develop better methods to improve the health and wellbeing of our community’s most vulnerable patients.

Our research interests include:

Population Based Research

Analyzing risk factors and longitudinal behavior of High-Need, High-Cost (HNHC) patients and HNHC sub-populations (i.e., frequent users of Emergency services) for development of predictive models.

Evaluation

Determining the efficacy of our Care Coordination through analysis of cost and patient pre/post survey data.

Systems and Data Integration

Bringing together medical and social organizations and designing data-driven interventions to assess and overcome barriers to HNHC patient care delivery and appropriate/effective use of medical and social resources.




Current Projects


We are partnering with researchers from the University of Southampton to design assessments for providers and patients to generate qualitative data needed to evaluate Care Coordination programs. This project aims to develop questionnaires to better understand how the continuum of care - including medical, behavioral and social services - is utilized, and how the experience could be improved for providers and patients alike.

We are collaborating with IMAGO Global Grassroots, afiliates of the Harvard Kennedy School and the University of San Diego, to develop highly effective partner relationships in a collaboartive services model.
