Who we are

We are a non-profit organization with a mission to improve quality and reduce cost through coordination of care for the most costly and vulnerable in our health care system.

What we do

We perform our mission in a variety of ways, from developing software and technology solutions for care coordination, to performing data analytics on large diverse datasets for identifying super utilizers, to setting up intervention programs and care plans for clients enrolled in the intervention program.

What we look for

We expect you to think outside the box in developing solutions to industry problems and work within a dynamic, collaborative, non-hierarchal, and ego-free culture where your talent and ideas are valued over a role title. We encourage you to take part in conferences, user groups, write blogs and enroll in continuing education and certification programs.
